After your form is in PDF you can actually create fields in Acrobat 7 Professional on top of your PDF enabling users to actually fill out the form right inside their web browsers with the Free Adobe Reader. If you have the Adobe Creative Suite 2 Premium, you have everything you need to create your own fillable forms.
